AVVC Forms

These forms give us the information we need to set up your record
in our management software so that we have all the information on
your account and your pet's health at our fingertips when you call.

Email, fax, or print and bring the applicable completed form to Abbott Valley Veterinary
Center.  You can deliver it before bringing your pet, or at the time of your check-in or
appointment - which ever is most convenient for you.

Supplemental reading:  For more information on your pets’ illness or diagnosis we refer clients to veterinarypartner.com

Poison Control:  In case of emergency toxin ingestion we refer to ASPCA Poison Control Hotline at (888) 426-4435 or
www.aspca.org  Please be prepared with a major credit card as there is a $60.00 fee for this service. If your pet has a Home
Again Microchip Identification and is registered there is no fee.

Professional Organizations: The Rhode Island Veterinary Medical Association website has great information about the
pet industry in general. You can find information about Animal and Public Health, Rhode Island Shelters and estimated costs
for owning a pet.

Pet Insurance: We recommend pet insurance to all of our clients. We do not recommend any specific insurance company
but can direct you to several reputable pet insurance carriers and their websites. Two examples are
VPI or ASPCA.

Grief Counseling and Support: When you have lost a beloved pet and are feeling emotionally overwhelmed there are
support services available. We refer our clients to
Cummings School of Veterinary Medicine at Tufts where you can find many
helpful links.

Obedience Training: We always recommend obedience training for all of our new puppies. Sometimes we may incorporate
obedience training into behavioral modification programs for our older patients. We recommend several options including
Canine Mastery in Seekonk MA. We also recommend Bark Busters for in-home training.

Kenneling Your Pet: If you are planning to travel and need a boarding kennel recommendation, please call our office and we
can advise you. If you are planning to travel with your pet we recommend
bringfido.com as a resource for hotels that
accommodate pets. The website is beneficial for all things travel related, when bringing along your pet.
